Pump Now Pay Later - Program Terms, Agreement and Subscription

TERMS AND CONDITIONS for Pump Now Pay Later^TM
Diabetes Express®, Bayshore Specialty Rx Ltd.

By selecting this item and depositing $1.00 against your account you (the Cardholder) hereby agree to all the terms and conditions listed below and to pay in full without delay, any and all funds to Diabetes Express®, Bayshore Specialty Rx Ltd., for the ordered and delivered supplies sent by Diabetes Express®. Full payment must be made to Diabetes Express®, Bayshore Specialty Rx Ltd., within 45 days of receiving the order once on the Pump Now Pay Later Program^TM

I understand and approve Diabetes Express® can and will charge the card used to confirm this agreement for the full remaining balance on my account.  I also accept that if my card should not approve the charge, Diabetes Express® will attempt an additional three (3) times to process the payment.

I agree and confirm that the payment option used today is not a pre-paid card.  I understand that if it is deemed to be a pre-paid card that Diabetes Express® can and will remove me from the program until proper billing methods are provided.  I also understand that this is in the sole discretion of Diabetes Express to extend the program if any infraction does occur.

REQUEST FOR CHANGE OF CHARGE CARD

If you would like to change the form of payment you must provide your new payment method to Diabetes Express®, Bayshore Specialty Rx Ltd., before the monthly installments are due.  

LOST/STOLEN CARDS

If your card has been lost or stolen you must contact  Diabetes Express®, Bayshore Specialty Rx Ltd. immediately and provide your new payment method to Diabetes Express®, Bayshore Specialty Rx Ltd., before the monthly installments are due in order to continue on the Pump now Pay Later Program^TM

FAILURE TO PAY

All Pump Now Pay Later Program^TM orders must be paid off on or before the 45 days terms,  in order to receive the next shipment and/or continue on the Pump Now Pay Later Program^TM.  If the transaction declines Diabetes Express®, Bayshore Specialty Rx Ltd., will notify you via email / phone when the transaction is not approved.  We require payment in full within the next 3 business days, failure to clear off the outstanding balance may affect 1) Any new orders going out  2) You eligibility to continue in the program.
