Subscribe and Save

This is a great way to save money and time plus get your FreeStyle Libre Sensors shipped automatically to your door.  By signing up for the Diabetes Express Subscribe and Save Program*  and selecting a start date, we will automatically send you four (4) four FreeStyle Libre 2 sensors at $97.00 each that are automatically shipped and billed every 56 days.

Please note:  In order for Diabetes Express to start your Subscribe and Save option you must pay via credit card at checkout - if any other options are selected the order will be voided.

TERMS AND CONDITIONS for the Subscribe and Save Program by Diabetes Express®

By selecting this item and depositing $1.00 against your account you (the Cardholder) hereby agree to all the terms and conditions listed below.

I agree to have Diabetes Express® ship four (4) Abbott FreeStyle Libre 2 sensors every 56 days starting the date I have indicated with this agreement at the cost of $97.00 CAD per sensor.

I understand and approve for Diabetes Express® can and will charge the card used to confirm this agreement for every future order until I indicate I wish to cancel the subscription.  I also accept that if my card should not approve the charge, Diabetes Express® will attempt an additional three (3) times to process the payment.

I understand that to cancel my subscription I will be required to contact Diabetes Express® a minimum of four business days prior to my next shipment.

REQUEST FOR CHANGE OF CHARGE CARD

If you would like to change the form of payment you must provide your new payment method to Diabetes Express®, Bayshore Specialty Rx Ltd., four business days before the next automatic shipment is due to be sent.  

LOST/STOLEN CARDS

If your card has been lost or stolen you must contact Diabetes Express®, Bayshore Specialty Rx Ltd. immediately and provide your new payment method to Diabetes Express®, Bayshore Specialty Rx Ltd., before the next automatic shipment is due to be sent.

FAILURE TO BILL

All Subscribe and Save orders must be paid prior to shipment.  If the transaction declines Diabetes Express®, Bayshore Specialty Rx Ltd., will notify you via email / phone when the transaction is not approved.  We require payment in full prior to the Subscribe and Save order to be shipped.
